Abstract

Purpose

Health is one of the most basic human rights. Self-medication not only results in many risks, complications and mortalities but also remains a massive economic burden on governments’ pharmaceutical budgets, insurance companies and the general population. The importance of self-medication as one of the worldwide health problems, this paper aims to investigate the extent of this problem and related factors in Sanandaj, Iran in 2018.

Design/methodology/approach

This cross-sectional, descriptive study was conducted on 838 people who referred to pharmacies in Sanandaj, Iran in 2018. The subjects were selected in 10 pharmacies, 84 in each, randomly. A self-constructed questionnaire was used to collect information about demographic features, prevalence and reasons for self-medication. The data was analyzed statistically by SPSS software.

Findings

The results show a high prevalence of self-medication, 73.70 per cent among people who referred to the pharmacies in Sanandaj during 2018. There was a significant correlation between self-medication and occupation, education level, age groups, chronic disease, smoking or reasons to refer to pharmacies (p < 0.05). Analgesics, antibiotics; non-steroid anti-inflammatory, cold and gastrointestinal medicines were the most commonly used medicines by the subjects. People with headache, cold, infection, stomach pain, toothache and dysmenorrhea were more likely to act self-medication than people with other complaints. Among people who do self-medication, (85.60 per cent) had already experienced satisfactory results after using medicines. Mild symptoms of the disease (74.11 per cent), availability of medicines (72.98 per cent) and easy access to medicine supplies in pharmacies (71.35 per cent) are other reasons for self-medication.

Research limitations/implications

In general, the study had some limitations. One of the limitations was related to the research period i.e. spring and summer. So, it is suggested to conduct studies in other seasons, too. Furthermore, as the time of self-medication was the past three months, there was a probability of recall bias.

Originality/value

The prevalence of self-medication in Sanandaj is high and alarming, implementing educational programs about this issue is extremely required. Health policymakers should take all necessary steps to tackle self-medication efficiently.

This study aims to investigate the health-care-seeking behaviour and practices of West African migrants who reside and operate in Wa, Ghana, as itinerant retailers.

Abstract

Purpose

This study aims to investigate the health-care-seeking behaviour and practices of West African migrants who reside and operate in Wa, Ghana, as itinerant retailers.

Design/methodology/approach

The study was cross-sectional and used the quantitative research approach. The analysis was done on a target population comprising 122 itinerant immigrant retail traders in Wa, Ghana. Fisher’s exact test and logistic regression were used to analyse the data.

Findings

Malaria was the commonest disease among them. Five in ten of the migrants preferred to report malaria episodes to a private health facility than to a government facility. Significant associations were identified between four dimensions (health facility, self-medication, home remedy and consult others) of health-seeking behaviour, and some background characteristics. The main reason why migrants prefer government health facilities was because of their better health personnel. They self-medicated because of easy accessibility of over-the-counter medicine shops. Also, when ill, the migrants usually consulted family members who would be in a position to take them home when their ailment worsens.

Research limitations/implications

Snowball sampling was used to select the respondents which could potentially lead to a sample that is not fully representative of the population in general.

Originality/value

Studies concerning migration and health in Ghana have been focused on internal migrants. Yet, minority immigrant traders equally encounter adverse health conditions but limited studies have been conducted to espouse their health-seeking behaviour. This study imperatively contributes to the subject matter that has limited literature in the country.

Abstract

Purpose

The over‐the‐counter (OTC) drug market is highly competitive, and consumer advertising is a prominent influence in OTC drug purchase and consumption. Given current marketplace conditions, it is important to summarize OTC drug advertising research. This paper aims to review the state of the public research literature on OTC drug advertising and provide a research agenda derived from the findings.

Design/methodology/approach

A literature review was conducted to identify the key themes in OTC drug advertising research and secondary data were collected about the regulation, nature, functions, and scope of OTC drug advertising.

Findings

Most pharmaceutical advertising studies have focused on prescription drugs, including the majority of direct‐to‐consumer advertising investigations. OTC drug advertising has received considerably less empirical attention. Since the mid‐1970s, only 24 OTC drug advertising studies have appeared sporadically in the literature. The cumulative findings are interesting and suggestive but dated, fragmented, and incomplete. Though research interest has waned, OTC drug markets and advertising spending have not. Advertising remains a prominent OTC drug purchase and consumption driver, likely spurred on by self‐medication and Rx‐to‐OTC drug switching. The state of the public research, the social and policy implications of self‐medication, and the growing OTC drug market signal that it is time to revisit OTC drug advertising content, processes, and effects.

Originality/value

The paper puts the subject of OTC drug advertising back on the radar of communication, advertising, and pharmaceutical marketing researchers and offers an agenda of research questions derived from the reviewed literature to guide and stimulate future studies.

Abstract

Purpose

Cannabis use continues to increase worldwide, and a number of nation states are changing their cannabis policies. Policy changes require research into key populations, namely, people who use cannabis. This study aims to examine sociodemographic differences of young Finns who reported using cannabis mainly for self-medication versus mainly recreationally, as well as their reported effects of cannabis use.

Design/methodology/approach

The data come from an anonymous online survey (N = 247, 70.0% males, 25.9% females, 4.1% other) that was analysed using multiple logistic regression. The authors focused on whether various demographic indicators differed between those who reportedly used cannabis mainly for recreational purposes and mainly for self-medicinal purposes. The authors also qualitatively examined the respondents’ experienced effects of cannabis, both desired and undesired.

Findings

Being older and female, living in a smaller city and earlier age of initiation of cannabis use were statistically significant in predicting the medicinal use of cannabis. The majority of recreational effects were related to themes such as relaxation and pleasure, but many participants also reported desired medical effects. Similarly, many participants reported several undesired effects.

Research limitations/implications

Understanding especially young people’s motivations to use cannabis, which include using it for various medical effects, can improve the design of harm reduction and treatment programmes as well as enhance the well-being of people who use cannabis.

Originality/value

This study gives a nuanced account of sociodemographic factors and motivations of young people who use cannabis in Finland as well as the reported effects it has on them, which complements data from national drug surveys.

Abstract

Purpose

The self-medication (SM) practice has been increasing in Europe, due to either an information policy for medicinal products or to highly educated people. The paper aims to consider a time constraint in the consumer decision optimization problem and test the optimal choices, using data from Portugal. Moreover, the author tries to understand the factors that determine the choice of SM.

Design/methodology/approach

First, the paper finds the optimal choices of a consumer using consumer theory. Then, the paper uses data from Portugal which is characterised by a universal public health system. The paper estimates a simple probit for SM. The estimation is run on adults, and also on gender and on age subsamples.

Findings

The main results show that waiting time is not a simple concept. While people may accept waiting a long time for an appointment and may not choose SM; waiting in the doctor's office is looked down upon and so choosing SM seems to be faster and more preferable. The results also show that there are some differences in the factors that explain SM according to age and gender, which matters for policy purposes.

Originality/value

The empirical work on SM in Europe is original. Taking into account a time constraint is new in the agent decision model of health demand.

Abstract

Purpose

This study aims at verifying the impact of literacy on over-the-counter (OTC) drug consumer vulnerability as evaluated by health literacy and label comprehension.

Design/methodology/approach

The item response theory (IRT) was used to estimate the health literacy of two groups and the two-way analysis of variance tests was used to test the hypotheses for the existence of mean differences between the two populations. The convenience sample involved 188 OTC consumers: 94 (50%) poorly literate and 94 (50%) university students/graduates.

Findings

University consumers/graduates have a level of health literacy and label comprehension that is superior to those presented by poor literate consumers. Also, age does not influence the level of health literacy by OTC drug users but has a significant impact on the understanding of OTC drug labels. Finally, the level of schooling and the “age group,” simultaneously, does not impact the understanding of OTC drug labels or health literacy.

Research limitations/implications

This study has added in the field of knowledge by investigating the behavior of poor literate consumers in Brazil, a developing country. The results may be relevant to Marketing professionals, especially those in the pharmaceutical industry, and to police makers, as they help identify the main problems faced by poorly literate consumers.

Practical implications

It is necessary to raise awareness of the dangers of self-medication and wrong use of medications, mainly focused on people with low literacy. As a suggestion, a simple glossary presented along with the label could provide explanations of scientific terms, thus increasing health literacy and reducing the vulnerability of the consumers.

Social implications

This study showed that when using common words such as gastritis to define a health problem, there is a higher degree of correctness. These results suggest the adoption of a more straightforward language and more precise explanations. By doing that, the pharmaceutical industry and policymakers will improve their social impact by increasing consumer power and taking care of the health of the most vulnerable population: the illiterate people.

Originality/value

This study contributes to the international literature, as it enhances and clarifies the knowledge about the customers’ power and vulnerability in developing countries. It fills a gap by evaluating label comprehension and heath literacy at the same time, giving an academic contribution for pharmaceutical consumers’ studies.

Purpose

Alcohol misuse and mental health problems in parents are both known to contribute to impaired outcomes in children, although little is known about the specific parenting behaviours that might be affected. Mental health problems in parents who misuse alcohol may impact parenting in specific ways, and these may be different for mothers and for fathers. The purpose of this paper is to make a preliminary investigation of alcohol misuse and mental health problems in mothers, and explore ways in which these might affect their parenting.

Design/methodology/approach

Semi-structured interviews were carried out with professionals involved in services for mothers who misuse alcohol, including those working in addiction psychiatry, in primary care, and in the provision of parenting services. These interviews were then examined using thematic analysis.

Findings

Themes emerged including specific mental health problems (depression, anxiety and personality disorders), and related issues, such as self-medication. Particularly relevant for mothers were post-natal depression, the effect on maternal alcohol misuse and mental health when children are removed, the role of domestic violence, and the importance for identification of home visits by services. Different types of alcohol misuse were linked to different mental states and different parenting behaviours in parents. Findings in the general parenting and substance misuse literature were confirmed, and a new addition was that expressed emotion may play a role in families where mothers misuse alcohol.

Originality/value

The findings increased the understanding of the relationships between maternal alcohol misuse, mental health issues and parenting, raising several new points for consideration.

Abstract

Purpose

The purpose of this study is to investigate the status of perceived need, seeking behavior and utilization of health services in the elderly population of Sanandaj (west of Iran).

Design/methodology/approach

This is a cross-sectional study conducted on 800 elderly people in Sanandaj. Subjects were selected using multistage sampling and data were collected using self-report questionnaires. A multivariate logistic model with odds ratios (ORs) was used to determine the relationship of independent variables with seeking perceived need. Also, the concentration index was used to measure the inequality in using health services.

Findings

The perceived need for outpatient (during the last 30 days) and inpatient health-care services (during the past 12 months) was 69.7% and 29.7%, respectively. Among them, the unmet need for outpatient and inpatient health-care services was 46.6% and 17%, respectively. Having health insurance (adjusted OR 12.08; 95% confidence interval [CI] 1.04–140.11), middle economic status (adjusted OR 5.18; 95% CI 1.30–20.51) and being in an age group of 65–70 years (adjusted OR 7.60; CI 1.42–40.61) increased the chance of seeking inpatient care. Also, being in an age group of 60–65 years (adjusted OR 0.41; 95% CI 0.18–0.95) reduced the chance of seeking outpatient care. There was also a pro-rich inequality in using outpatient health services.

Originality/value

The elderly population suffers from unmet health-care needs, especially in outpatient services. The most important reason for not seeking outpatient and inpatient services was financial barriers and self-medication, respectively. So, designing targeted policies and interventions to address barriers in the conversion of need to demand in the elderly population is essential.

Abstract

Purpose

The aim of this paper is to explore, through exploratory qualitative research, how perceptions of involvement and risk, for female consumers, influences their information search and product evaluation decision‐making for over‐the‐counter (OTC) painkillers.

Design/methodology/approach

In‐depth interviews were conducted amongst 25 female OTC painkiller purchasers from one pharmacy. Females are more engaged with and have a greater likelihood to use OTC medication. Despite the benefits of applying qualitative approaches when researching self‐medication, there are few self‐care qualitative studies.

Findings

Despite the greater availability and awareness of OTC medicines, the purchasing process is still underpinned by inherent risk. Alongside base levels of involvement, painkillers had situational importance with there being unease as to the risk involved. Consumers felt uncertainty as to the worth of their knowledge and this was compounded by a lack of informational clarity. Finally, the risk of side‐effects and the relative importance of subjective beliefs often took precedence during evaluation.

Research limitations/implications

Information needs to be made more accessible, whilst there is scope to capitalise upon family history as interpersonal sources were deemed to be empathetic. As the study was conducted once in a “local” setting, the time span and level of interactivity could be extended by studying consumers “lived experiences”. Triangulated research amongst related parties, such as pharmacists, could build on this exploratory study.

Originality/value

OTC medicine sales have grown with self‐medicating consumers purchasing painkillers the most frequently. Previous research has stemmed from medical sociology, pharmacy practice and public policy, and there is a lack of contemporary (UK) consumer behaviour research on OTC painkiller purchasing.

Abstract

Purpose

Cyberchondria refers to the repeated and excessive search for health-related information online, associated with increased health anxiety. This paper utilizes the protection motivation theory to investigate the negative behavioral consequences of cyberchondria that pose health risks to users, such as trust in the physician, propensity to self-medicate, and therapy compliance.

Design/methodology/approach

The data for the study were collected from a sample of 317 participants in India using an online survey and form. The analysis was conducted using structural equation modeling.

Findings

Cyberchondria negatively affects the trust in physician and positively affects the propensity to self-medicate. Trust in physician negatively affects the propensity to self-medicate and positively affects therapy compliance. Furthermore, trust in physician partially mediates the relationship between cyberchondria and the propensity to self-medicate and completely mediates the relationship between cyberchondria and therapy compliance. Cyberchondria has no direct significant effect on therapy compliance.

Research limitations/implications

Researchers need to examine other behavioral or psychological factors affected by the reduced trust in physicians due to cyberchondria.

Practical implications

Physicians and health care providers should refocus on patients with cyberchondria and regain their trust through quality interactions and services. Policymakers may consider regulating online health information publication to set the standards of information quality and source. Websites and platforms publishing health information online should distinctly label verified information.

Originality/value

This study investigates the damaging effects of cyberchondria's behavioral consequences that pose health risks to users.
